• Name of firm, Category and dosage forms of Product, Site Plan and Layout of Building as per schedule Consent letter from respective state Pollution Control Board, List of technical staff with qualification and experience. List of machinery, AHU Installation And Validation Certificate (Wherever Necessary), Water System Installation And Validation Certificate (Wherever Necessary).



Drug Manufacturing(Loan)

• Loan license means a license which the Licensing Authority may issue to an applicant who does not have his own arrangements for manufacture but who intends to avail himself of the manufacturing facilities owned by a licensee .
• Name of the establishment with license copy , Name of the Firm with authorized person (Applicant), Category and dosage forms of Product, List of technical staff with qualification and experience. List of machinery, AHU Installation And Validation Certificate (Wherever Necessary), Water System Installation And Validation Certificate (Wherever Necessary).
